No pass or fail, only bands
The British Council says you cannot pass or fail IELTS: you get a band score between 0 and 9, and each educational institution, government or organisation sets its own minimum (British Council). IDP India puts it the same way when asked whether 4.5 is a fail: IELTS has no pass or fail, and whether 4.5 is enough depends on the requirement you are trying to meet (IDP India).
IELTS for UKVI Life Skills (A1, A2, B1) is different: its Test Report Form shows PASS or FAIL, and a fail cannot be used for a visa application (British Council). See our Life Skills guide.
What each band means
ielts.org gives every band a skill level and a short description (ielts.org).

So what score counts as a pass for me?
Whatever your target organisation publishes. The British Council's FAQ gives some typical ranges, while stressing that every institution sets its own: UK undergraduate degrees usually ask for a minimum of 5.5–6.5, with more for postgraduate study; most European universities ask for 5.5–7.0; and visa requirements across English-speaking countries range from about band 4 to 7.5 (British Council).
Many requirements set a minimum overall band and a minimum in each skill, so a high overall can still fail one condition. Check both numbers.

How the overall band is worked out
The overall band is the average of the four skill bands, rounded to the nearest half band. An average ending in .25 rounds up to the next half band and one ending in .75 rounds up to the next whole band, so 6.25 becomes 6.5 and 6.125 becomes 6.0 (ielts.org). What is the passing score for IELTS?+
There isn't one. IELTS reports bands from 0 to 9, and the organisation you apply to decides what score it needs.
Is 4.5 a fail in IELTS?+
No. IELTS has no fail. Whether 4.5 is enough depends on the requirement you are trying to meet.
Is 6.5 a good IELTS score?+
ielts.org calls band 6 'Competent' and band 7 'Good'. 6.5 meets many undergraduate and some postgraduate requirements, but check your course.
Can you fail IELTS?+
Not the Academic or General Training test. Only IELTS for UKVI Life Skills gives a Pass or Fail result.
What does band 0 mean in IELTS?+
ielts.org uses band 0 for 'did not attempt the test': the test taker did not answer the questions.
