Evidence Pearson needs
Extra time, a break or a separate room is granted only with medical evidence that meets all of these points:
A letter or certificate from a licensed medical practitioner, written when you were 12 or older (the most recent you have); on official letterhead with the practitioner's name, title, credentials, practice details, date and signature; showing a disability with a substantial and long-term adverse effect that requires the accommodation, with full details of any learning difficulty and the official assessments carried out; and in English, or professionally translated with proof of the translator's qualification.
Pearson may ask for more evidence and can revoke scores or ban test takers who use false evidence (accommodations policy).

How to request
Tick the Special Test Arrangements option when you create your myPTE account, then complete the request form. Customer Support reviews it and books your test for you. Submit it at least six weeks before your preferred date; it can take longer if more documents are needed (accommodations policy; help centre).
If a hand injury would stop the palm vein scan, contact Pearson before booking, or you will be turned away and lose your fee.
PTE is computer-delivered and computer-scored, so Pearson warns that it may not suit every test taker, and no score adjustment is made for unexpected illness on the day.
Comfort aids: no request needed
You may bring these into the test room once the administrator has checked them: tissues; unwrapped cough drops; a pillow to support your neck, back or an injured limb; a sweater, sweatshirt or blazer (show that the pockets are empty); glasses and hearing aids; neck braces or collars; and an insulin pump attached to your body. Bring your own.
For comparison, IELTS offers a wider range, including Braille papers and skill exemptions; see IELTS access arrangements.
Pearson updates its policies from time to time. Check the linked policy page before you act. This guide is general information, not official PTE advice.
